# Oxymetazoline Nasal Spray (Sinex)
## Overview
Oxymetazoline is a topical vasoconstrictor and decongestant.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* Instill 2-3 sprays in each nostril every 10-12 hours as needed.
* Do not exceed 2 doses in a 24-hour period.
* Limit use to a maximum of 3 consecutive days.
## Pediatric Dosing
* Children 6-12 years: Instill 2-3 sprays in each nostril every 10-12 hours as needed. Limit use to a maximum of 3 consecutive days.
* Children under 6 years: Safety and efficacy not established. Consult prescribing information or local protocol for specific guidance.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients receiving or within 2 weeks of receiving MAO inhibitors.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing, increased nasal discharge.
* **Serious:** Rhinitis medicamentosa (rebound congestion) with prolonged or excessive use, systemic effects (hypertension, arrhythmias, nervousness, insomnia) are rare with topical use but possible.
## Key Drug Interactions
* Monoamine oxidase inhibitors (MAOIs): Concomitant use or use within 2 weeks of MAOI therapy can lead to hypertensive crisis.
* Other vasoconstrictors: Potential for additive pressor effects.
## Monitoring
* Monitor for signs and symptoms of rebound congestion (rhinitis medicamentosa) with prolonged use.
* Assess for systemic cardiovascular effects, especially in patients with pre-existing cardiovascular disease, if overuse is suspected.
## Clinical Pearls
* Limit use to 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* If congestion does not improve after 3 days, discontinue use and consult a healthcare provider.
* Ensure the nasal spray tip does not touch the nasal mucosa to prevent contamination.
