# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of the nasal mucosa. This reduces swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergies, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 years and older:** 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended. Consult a pediatrician.
## Dose Adjustments
* No dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal with topical use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with caution in patients with cardiovascular disease (hypertension, ischemic heart disease), hyperthyroidism, diabetes mellitus, or prostatic hypertrophy.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (>3 days), leading to worsening congestion.
* **Systemic Effects (rare with topical use but possible with overuse):** Headache, insomnia, nervousness, increased blood pressure, tachycardia.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can lead to hypertensive crisis. Avoid use within 14 days of MAO inhibitor therapy.
* **Tricyclic Antidepressants:** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion if used for more than 3 days.
* Monitor blood pressure and heart rate if systemic absorption is suspected, especially in patients with underlying cardiovascular conditions or with excessive use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* If congestion is not relieved after 3 days, or if symptoms worsen, discontinue use and consult a healthcare professional.
* Shake well before use.
* Avoid touching the applicator tip to the nostril to prevent contamination.
