# Sinex (Oxymetazoline)
## Overview
Sinex is a topical nasal decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* Administer 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* Do not exceed 2 doses in any 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 2 to 3 sprays in each nostril every 10 to 12 hours as needed.
* **Children under 6 years:** Not recommended. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients receiving or have recently received MAO inhibitors.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased mucus production, rebound congestion (rhinitis medicamentosa) with prolonged use.
* **Less common:** Dizziness, headache, nervousness, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use can lead to a hypertensive crisis. Avoid use in patients receiving MAO inhibitors or within 2 weeks of discontinuing them.
* **Beta-blockers:** May potentiate the pressor response.
* **Tricyclic Antidepressants:** May potentiate the pressor response.
## Monitoring
* Monitor for signs and symptoms of rebound congestion (rhinitis medicamentosa), especially with prolonged or frequent use.
* Monitor blood pressure, particularly in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* If symptoms persist, discontinue use and consult a healthcare provider.
* Shake well before each use.
* Avoid touching the nasal applicator tip to the nostrils to prevent contamination.
