# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal vessels, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 applications in a 24-hour period. Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended; consult a pediatrician.
* **Maximum:** Do not exceed 2 applications in a 24-hour period. Do not use for more than 3 consecutive days.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment for topical nasal use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, irritation. Sneezing. Rebound congestion (rhinitis medicamentosa) with prolonged use.
* **Less Common:** Headache, nervousness, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use can lead to hypertensive crisis. Discontinue MAOIs at least 14 days before starting oxymetazoline.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion or overuse.
* Monitor blood pressure in patients with pre-existing hypertension or those using MAOIs or TCAs.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* If symptoms do not improve or worsen, discontinue use and consult a healthcare provider.
* Prime the pump mechanism before first use by actuating it several times.
