# Oxymetazoline Nasal Spray (e.g., Sinex)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that acts as a vasoconstrictor. When applied to the nasal mucosa, it reduces swelling and mucus production, providing temporary relief of nasal congestion.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
* Temporary relief of sinus congestion and pressure.
## Adult Dosing
* **Dose:** 2 sprays in each nostril.
* **Frequency:** Every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Ages 6-11 years:** 1-2 sprays in each nostril.
* **Frequency:** Every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
* **Children under 6 years:** Not recommended; consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically needed for renal or hepatic impairment when used as directed for short durations.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having stopped within the last 2 weeks MAO inhibitors.
* Risk of narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal discomfort, dryness, sneezing, burning sensation, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic vasoconstriction (hypertension, tachycardia, palpitations), headache, nervousness, insomnia, blurred vision.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use or use within 2 weeks of MAO inhibitors can cause severe hypertensive crisis.
* **Other Vasoconstrictors:** Use with caution as it may potentiate systemic vasoconstrictive effects.
## Monitoring
* Monitor for signs and symptoms of rebound congestion (worsening nasal congestion after discontinuation).
* Monitor blood pressure and heart rate, especially in patients with cardiovascular disease or those using the medication frequently or for extended periods.
* Assess for signs of systemic absorption, particularly in pediatric patients or those with underlying conditions.
## Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa (rebound congestion), which can lead to chronic nasal congestion and dependence.
* Advise patients to blow their nose gently before application.
* Keep the nozzle clean and avoid sharing the nasal spray to prevent infection transmission.
* Store at room temperature, away from heat and direct sunlight.
