# Sinex (Oxymetazoline)
## Overview
Sinex is a topical nasal decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* Instill 2 or 3 sprays into each nostril every 10-12 hours as needed.
* Do not exceed 2 doses in a 24-hour period.
* Limit use to a maximum of 3 consecutive days.
## Pediatric Dosing
* **Children 6 to under 12 years:** Instill 2 or 3 sprays into each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period. Limit use to a maximum of 3 consecutive days.
* **Children under 6 years:** Safety and efficacy have not been established. Consult a pediatrician.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption. However, caution is advised in patients with severe cardiovascular disease or uncontrolled hypertension.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of stopping an MAO inhibitor.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge, headache.
* **Rebound Congestion (Rhinitis Medicamentosa):** Prolonged or excessive use can lead to chronic nasal congestion that is difficult to treat. This is the most significant concern with topical nasal decongestants.
* **Systemic Effects (rare with proper use):** Increased blood pressure, palpitations, nervousness, insomnia.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use can lead to a hypertensive crisis. Avoid use within 14 days of stopping MAOIs.
* **Beta-blockers:** May enhance the hypertensive effect.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor response.
## Monitoring
* Monitor for signs of rebound congestion (worsening congestion after initial relief).
* Assess for cardiovascular side effects (e.g., elevated blood pressure, palpitations) in susceptible individuals.
## Clinical Pearls
* Sinex is intended for short-term use only (maximum 3 days).
* Educate patients on the risk and management of rhinitis medicamentosa.
* If symptoms persist beyond 3 days, advise patients to discontinue Sinex and consult a healthcare provider.
* Consider alternative treatments like nasal saline rinses or intranasal corticosteroids for chronic congestion.
