# Oxymetazoline (Sinex)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
* **Children 6-12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
* **Children under 6 years:** Safety and efficacy have not been established. Consult a healthcare provider.
## Dose Adjustments
* No dose adjustments are typically needed based on organ dysfunction.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the past 14 days.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (>3-5 days). This leads to worsening congestion upon cessation of the drug.
* **Systemic Effects (rare with topical use):** Insomnia, nervousness, dizziness, headache, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use or use within 14 days can lead to a hypertensive crisis.
* **Other Vasoconstrictors:** Potential for additive pressor effects.
## Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure and heart rate if systemic absorption is suspected, particularly in sensitive individuals or with excessive use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* Advise patients to use the lowest effective dose and frequency.
* If symptoms do not improve or worsen, advise discontinuation and consultation with a healthcare provider.
* Consider alternative treatments for chronic nasal congestion.
