# Oxymetazoline (Sinex)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of the nasal mucosa.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children under 6 years:** Consult a pediatrician. Safety and efficacy are not established.
## Dose Adjustments
* No dose adjustments are typically needed for renal or hepatic impairment due to minimal systemic absorption when used as directed.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken MAO inhibitors within the last 14 days (risk of hypertensive crisis).
## Adverse Effects
* **Common:** Nasal discomfort, dryness, burning, stinging, sneezing.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, increased blood pressure, insomnia, nervousness, dizziness, palpitations.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use can lead to severe hypertension. Avoid use if patient is on or recently has been on MAOIs.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May mask reflex tachycardia and can lead to unopposed alpha-stimulation if systemic absorption occurs.
## Monitoring
* Monitor for signs of rebound congestion (worsening nasal congestion after initial relief, especially after stopping the medication).
* Monitor blood pressure in patients with pre-existing hypertension, especially with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rhinitis medicamentosa.
* Advise patients to blow their nose before application to ensure proper delivery.
* Do not share nasal spray devices.
