# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction. This reduces swelling and congestion of nasal passages.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
Administer 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
**Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
**Children under 6 years:** Consult a healthcare provider.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment as systemic absorption is minimal.
## Contraindications
Hypersensitivity to oxymetazoline or any component of the formulation. Patients with narrow-angle glaucoma.
## Adverse Effects
Common: Nasal discomfort, burning, stinging, sneezing.
Less Common: Rebound congestion (rhinitis medicamentosa) with prolonged use, insomnia, nervousness, dizziness, palpitations.
## Key Drug Interactions
Monoamine oxidase inhibitors (MAOIs) and within 14 days of stopping MAOIs: May cause hypertensive crisis.
Other sympathomimetic agents: May potentiate systemic effects.
## Monitoring
Monitor for signs of rebound congestion (rhinitis medicamentosa) if used for more than 3 consecutive days. Assess for systemic side effects like increased heart rate or blood pressure, especially in patients with cardiovascular disease.
## Clinical Pearls
Limit use to a maximum of 3 consecutive days to avoid rebound congestion.
Shake well before use.
Prime the pump by spraying several times if the product has not been used or if the spray mechanism is clogged.
Use caution in patients with cardiovascular disease, hypertension, diabetes, hyperthyroidism, or prostatic hyperplasia.
