# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction in the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray (0.05%):** Administer 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician guidance. Safety and efficacy are not established.
## Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use in children under 6 years of age is generally avoided for the 0.05% formulation without physician guidance.
## Adverse Effects
* **Common:** Nasal stinging, burning, dryness, sneezing, transient irritation.
* **Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use (more than 3 days), potentially leading to chronic nasal obstruction. Systemic effects (rare) can include increased blood pressure, headache, nervousness, insomnia, palpitations.
## Key Drug Interactions

* **Beta-blockers:** May mask the systemic effects of oxymetazoline (e.g., reflex tachycardia).
* **Tricyclic Antidepressants (TCAs):** May potentiate the hypertensive effects.
## Monitoring
* Monitor for signs of rebound congestion or over-use.
* Assess for systemic adrenergic effects, especially in patients with cardiovascular disease or hypertension.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
