# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist, causing vasoconstriction of nasal mucosal blood vessels. This reduces swelling and congestion.
## Primary Indications
Symptomatic relief of nasal congestion due to the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
0.05% nasal spray: Two sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
## Pediatric Dosing
* **6 to under 12 years:** 0.05% nasal spray: One spray in each nostril every 10-12 hours as needed. Do not exceed 2 applications in a 24-hour period.
* **Under 6 years:** Use is generally not recommended without physician guidance due to lack of established dosing and increased risk of adverse effects.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use with MAO inhibitors or within 14 days of discontinuing MAO inhibitors due to risk of hypertensive crisis.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing.
Less common: Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, nervousness, insomnia.
## Key Drug Interactions
* MAO inhibitors: Risk of hypertensive crisis.
* Beta-blockers: May potentiate pressor effects.
* Tricyclic antidepressants: May potentiate pressor effects.
## Monitoring
Monitor for signs of rebound congestion, especially with use exceeding the recommended duration. Monitor blood pressure in patients with hypertension, particularly with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* Prime the pump before first use and after periods of disuse.
* Avoid touching the nozzle tip to nasal passages to prevent contamination.
* Use cautiously in patients with cardiovascular disease, hypertension, diabetes, hyperthyroidism, and glaucoma.
