# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa.
## Primary Indications
* Nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **2 sprays** in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 applications per nostril in a 24-hour period.
## Pediatric Dosing
* **Ages 6-11 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Ages under 6 years:** Use is not recommended without physician guidance.
## Dose Adjustments
No dose adjustments are typically necessary for renal or hepatic impairment, given minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or who have taken monoamine oxidase inhibitors (MAOIs) within the past 14 days.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, rebound congestion (rhinitis medicamentosa).
* **Less Common:** Headache, insomnia, nervousness, palpitations.
## Key Drug Interactions
* **MAOIs:** Concomitant use can lead to severe hypertension.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* If symptoms persist beyond 3 days, discontinue use and consult a healthcare provider.
* Store at room temperature, away from heat and direct sunlight.
