# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 applications in 24 hours.
* **Children under 6 years:** Use is generally not recommended without physician guidance due to risk of systemic absorption.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment when used as a nasal spray due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use with MAO inhibitors or within 14 days of discontinuing MAO inhibitors (risk of hypertensive crisis).
* Patients with narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (>3 days consecutively). Symptoms include worsening congestion, nasal stuffiness, and dependence on the spray.
* **Systemic effects (rare with nasal use):** Headache, nervousness, dizziness, increased blood pressure, palpitations, insomnia.
## Key Drug Interactions
* **MAO Inhibitors (and within 14 days of stopping):** Potentiates hypertensive effects.
* **Tricyclic Antidepressants:** May potentiate hypertensive effects.
## Monitoring
* Monitor for signs and symptoms of rebound congestion.
* Monitor blood pressure, especially in patients with pre-existing hypertension or cardiovascular disease.
* Monitor for systemic side effects with excessive or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion (rhinitis medicamentosa).
* Instruct patients to blow their nose gently before application.
* Keep the tip of the spray nozzle clean and avoid contact with eyes.
* Store at room temperature.
