# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical nasal decongestant, an alpha-adrenergic agonist that causes vasoconstriction of the nasal mucosa, reducing swelling and congestion. It is available in various strengths and formulations, including nasal sprays.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray (0.05%):** Typically 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Ages 6-11 years (0.05%):** Typically 1-2 sprays in each nostril every 10-12 hours as needed.
* **Ages under 6 years:** Not recommended unless specifically directed by a healthcare provider. Consult specific product labeling for age restrictions.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Concurrent use with MAO inhibitors or within 14 days of discontinuing MAO inhibitors.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, transient nasal irritation.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged use, increased blood pressure, insomnia, nervousness.
## Key Drug Interactions
* **MAO Inhibitors:** Potential for hypertensive crisis. Avoid concurrent use.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline. Monitor blood pressure.
* **Beta-Blockers:** May reduce the local effect of oxymetazoline and potentially lead to reflex tachycardia.
## Monitoring
* Monitor for signs of rebound congestion (increased congestion upon cessation of use).
* In patients with cardiovascular disease or uncontrolled hypertension, monitor blood pressure.
## Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa.
* Instruct patients on proper nasal spray technique to ensure adequate delivery and minimize systemic absorption.
* If symptoms worsen or persist after 3 days, advise patients to discontinue use and consult a healthcare provider.
