# Oxymetazoline Nasal Spray
## Overview
Oxymetazoline is a topical decongestant that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Sinex (0.05% solution):** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years of age (Sinex 0.05%):** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years of age:** Not recommended. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Patients who have undergone transsphenoidal hypophysectomy or nasal surgery where the dura mater may have been exposed.
## Adverse Effects
* **Common:** Nasal discomfort, stinging, burning, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, palpitations, nervousness, insomnia) are rare with topical use but possible with overuse.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use can lead to hypertensive crisis. Avoid use within 14 days of MAOI discontinuation.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
## Monitoring
* Monitor for rebound congestion with prolonged or excessive use.
* Monitor blood pressure in patients with pre-existing cardiovascular conditions, especially with significant systemic absorption (though rare).
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rhinitis medicamentosa.
* Instruct patients on proper administration technique to maximize efficacy and minimize systemic absorption.
* Shake well before each use.
