# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor. It reduces nasal swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 administrations in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Safety and efficacy not established. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically necessary for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or having taken within the past 14 days MAO inhibitors.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (typically > 3-5 days). This can lead to chronic nasal obstruction requiring cessation of the medication and potential treatment for withdrawal.
* **Systemic Effects (rare with topical use):** Headache, insomnia, nervousness, increased blood pressure, palpitations.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concomitant use or use within 14 days of MAOIs can cause a hypertensive crisis.
* **Other Sympathomimetic Agents:** May potentiate systemic effects such as increased blood pressure.
## Monitoring
* Monitor for signs of rebound congestion (rhinitis medicamentosa) with prolonged use.
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions, especially with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rebound congestion.
* Educate patients on proper administration technique to ensure effective delivery and reduce systemic absorption.
* If symptoms do not improve after 3 days or worsen, discontinue use and consult a healthcare professional.
