# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical decongestant. It causes vasoconstriction of the nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 administrations in a 24-hour period.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Use is not recommended; consult a pediatrician.
* **Maximum:** Do not exceed 2 administrations in a 24-hour period.
## Dose Adjustments
No dose adjustments are typically needed based on renal or hepatic impairment for topical nasal use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients taking or having taken MAO inhibitors within the past 14 days (risk of hypertensive crisis).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, irritation. Sneezing.
* **Less Common/Serious:** Rhinitis medicamentosa (rebound congestion with prolonged use), palpitations, increased blood pressure, insomnia, nervousness.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use or recent use (within 14 days) can lead to severe hypertension.
* **Other Sympathomimetics:** May potentiate cardiovascular effects.
## Monitoring
* Monitor for signs of rhinitis medicamentosa, particularly with prolonged or frequent use.
* Monitor blood pressure in patients with pre-existing hypertension or cardiovascular disease, especially with frequent or excessive use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* Administer nasal spray with the head held upright.
* Prime the pump before first use and if the device has not been used for several days.
* Blow nose gently before administration.
* Wipe the tip of the nozzle after use.
