# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor. It reduces swelling and congestion in the nasal passages.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** Instill 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in a 24-hour period. Do not use for more than 3 consecutive days.
* **Children under 6 years:** Safety and efficacy have not been established. Consult a pediatrician.
## Dose Adjustments
No dose adjustments are typically necessary based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Infants and children under 6 years of age (unless specifically directed by a physician for certain products).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, nervousness, insomnia, palpitations, hypertension (especially with systemic absorption or overdose).
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use may cause a hypertensive crisis. Avoid use within 14 days of MAOI discontinuation.
* **Beta-blockers:** May increase the risk of systemic vasoconstriction and hypertension.
* **Tricyclic Antidepressants:** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for signs of rebound congestion (worsening congestion after initial relief, often with increased frequency of use).
* Monitor blood pressure in patients with pre-existing cardiovascular conditions, especially with frequent or prolonged use.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rhinitis medicamentosa.
* Use with caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes, or prostatic hypertrophy.
* Advise patients to blow their nose gently after application to distribute the medication.
