# Oxymetazoline (Sinex)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 0.05% solution.
* Administer 2-3 sprays in each nostril every 10-12 hours.
* Do not exceed 2 doses in a 24-hour period.
* Limit use to a maximum of 3 consecutive days to avoid rebound congestion.
## Pediatric Dosing
* **Children 6-11 years:** 0.05% solution.
* Administer 2-3 sprays in each nostril every 10-12 hours.
* Do not exceed 2 doses in a 24-hour period.
* Limit use to a maximum of 3 consecutive days.
* **Children under 6 years:** Use is not established. Consult a pediatrician.
## Dose Adjustments
* No dose adjustments are typically needed based on renal or hepatic impairment, given the minimal systemic absorption when used as directed.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients currently taking or within 14 days of stopping monoamine oxidase inhibitors (MAOIs).
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Rebound nasal congestion (rhinitis medicamentosa), increased blood pressure, headache, insomnia.
* **Rare:** Systemic effects (e.g., dizziness, increased heart rate) may occur, particularly with overuse or in sensitive individuals.
## Key Drug Interactions
* **MAOIs (including isocarboxazid, phenelzine, tranylcypromine) and within 14 days of discontinuation:** Potential for severe hypertensive crisis.
* **Other sympathomimetic agents:** May potentiate cardiovascular effects (e.g., increased blood pressure, heart rate).
## Monitoring
* Monitor for signs of rebound congestion or overuse.
* Assess for systemic side effects, especially in patients with cardiovascular disease or those using other sympathomimetic agents.
## Clinical Pearls
* Oxymetazoline is for topical nasal use only. Avoid contact with eyes.
* Prolonged or excessive use can lead to rhinitis medicamentosa, characterized by worsening congestion and dependence on the spray.
* Advise patients to use only as directed and for the shortest duration necessary.
* If symptoms persist after 3 days of use, discontinue and consult a healthcare provider.
