# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** Typically 0.05% solution. Administer 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 administrations in a 24-hour period. Limit use to 3 consecutive days.
## Pediatric Dosing
* **Nasal Spray (0.05%):**
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Use is generally not recommended; consult a pediatrician.
* **Maximum:** Do not exceed 2 administrations in a 24-hour period. Limit use to 3 consecutive days.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use within 14 days of stopping a monoamine oxidase inhibitor (MAOI) or within 2 weeks of discontinuing certain other sympathomimetic agents.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, dryness, sneezing.
* **Rebound congestion (rhinitis medicamentosa):** Occurs with prolonged or excessive use (>3 days), leading to worsening congestion upon cessation.
* **Systemic effects (rare with topical use):** Hypertension, headache, nervousness, insomnia, palpitations, tachycardia.
## Key Drug Interactions
* **MAOIs and other sympathomimetics:** Increased risk of hypertensive crisis. Avoid concurrent use.
* **Tricyclic antidepressants (TCAs):** May potentiate pressor effects. Monitor blood pressure.
## Monitoring
* Assess for improvement in nasal congestion.
* Monitor for signs of rebound congestion or systemic adverse effects with prolonged use.
* Check blood pressure in patients with pre-existing hypertension or those using concurrent interacting medications.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If congestion persists after 3 days, reassess the diagnosis and consider alternative treatments.
* Proper nasal spray technique is important for efficacy and to minimize systemic absorption.
* Store at room temperature.
