# Sinex (oxymetazoline)
## Overview
Sinex is a topical nasal decongestant spray containing oxymetazoline, an alpha-adrenergic agonist. It causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
Temporary relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
**Nasal Spray:** Instill 2-3 sprays into each nostril every 10-12 hours as needed.
**Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
**Children 6 years and older:** Instill 2-3 sprays into each nostril every 10-12 hours as needed.
**Children under 6 years:** Not recommended; consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment for topical nasal use.
## Contraindications
Hypersensitivity to oxymetazoline or any component of the formulation. Patients with narrow-angle glaucoma.
## Adverse Effects
Common: Nasal burning, stinging, dryness, sneezing.
Less Common: Increased blood pressure, headache, dizziness, insomnia, palpitations.
Serious: Rebound congestion (rhinitis medicamentosa) with prolonged use.
## Key Drug Interactions
Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 14 days of MAOI discontinuation may cause a hypertensive crisis.
## Monitoring
Monitor for signs of rebound congestion with prolonged use. Blood pressure monitoring may be considered in patients with hypertension.
## Clinical Pearls
Limit use to a maximum of 3 days. If congestion persists, consider alternative treatments and consult a healthcare provider. Do not share nasal spray bottles.
