# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Dose:** Do not exceed 2 administrations in a 24-hour period. Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Safety and efficacy not established. Consult pediatrician.
* **Maximum Dose (Children 6-12):** Do not exceed 2 administrations in a 24-hour period. Do not use for more than 3 consecutive days.
## Dose Adjustments
No dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Patients who have undergone transsphenoidal hypophysectomy or other transnasal surgery where the dura mater may have been exposed.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Rebound Congestion (Rhinitis Medicamentosa):** Occurs with prolonged or excessive use (more than 3 days).
* **Less Common:** Dizziness, headache, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **Monoamine Oxidase Inhibitors (MAOIs):** Concurrent use can potentiate the pressor effect of oxymetazoline. Avoid use within 2 weeks of discontinuing MAOIs.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effect of oxymetazoline.
## Monitoring
* Monitor for rebound congestion and limit use to 3 days.
* Monitor blood pressure, especially in patients with pre-existing hypertension or those taking other sympathomimetic agents.
## Clinical Pearls
* Limit use to 3 consecutive days to prevent rhinitis medicamentosa.
* If congestion persists beyond 3 days, advise patients to discontinue oxymetazoline and consult a healthcare provider.
* Avoid spraying directly on the nasal septum.
