# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that causes vasoconstriction, leading to reduced nasal mucosal swelling and congestion. It is a topical nasal decongestant.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 years and older:** 1-2 sprays in each nostril every 10-12 hours as needed. Do not exceed 2 doses in 24 hours.
* **Children younger than 6 years:** Safety and efficacy are not established. Consult a pediatrician.
## Dose Adjustments
* No dose adjustments are typically necessary for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with MAO inhibitors or within 14 days of discontinuing MAO inhibitors due to risk of hypertensive crisis.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Less Common:** Rebound congestion (rhinitis medicamentosa) with prolonged use, headache, dizziness, insomnia, palpitations, increased blood pressure.
## Key Drug Interactions
* **MAO Inhibitors:** Concurrent use or recent discontinuation can lead to hypertensive crisis.
* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May blunt the hypotensive effects of beta-blockers and increase the risk of systemic alpha-adrenergic effects.
## Monitoring
* Monitor for signs of rebound congestion (worsening nasal congestion after initial relief, increased frequency of use).
* Monitor blood pressure, especially in patients with pre-existing hypertension or those taking other sympathomimetic agents.
## Clinical Pearls
* Limit use to a maximum of 3 days to prevent rebound congestion.
* Proper administration technique (e.g., tilting head forward) may improve delivery and reduce systemic absorption.
* Advise patients to consult a healthcare provider if symptoms do not improve or worsen.
