# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal vasculature, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6 to 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Not recommended without physician guidance. Safety and efficacy have not been established.
## Dose Adjustments
No dose adjustments are typically needed based on renal or hepatic impairment for topical nasal use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Use with MAO inhibitors or within 2 weeks of stopping MAO inhibitors.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common:** Headache, dizziness, nausea, palpitations, insomnia.
* **Serious (with overuse):** Rebound congestion (rhinitis medicamentosa), systemic absorption leading to hypertension, tachycardia, CNS depression, especially in children.
## Key Drug Interactions
* **MAO Inhibitors:** Concomitant use can lead to hypertensive crisis.
* **Tricyclic Antidepressants:** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May mask reflex tachycardia.
## Monitoring
* Monitor for signs of rebound congestion (worsening congestion upon discontinuation).
* Monitor blood pressure and heart rate, particularly in patients with cardiovascular disease or with suspected systemic absorption.
## Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa.
* Administer by tilting head slightly forward and squeezing bottle gently.
* Prime pump before first use or if not used for several days.
* Store at room temperature.
