# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical vasoconstrictor. It is primarily used in nasal sprays to relieve nasal congestion.
## Primary Indications
* Nasal congestion associated with the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours.
* **Maximum Daily Dose:** Not explicitly defined by standard guidelines for intermittent use, but prolonged or excessive use is discouraged.
## Pediatric Dosing

* **Tricyclic Antidepressants (TCAs):** May potentiate the pressor response.
## Monitoring
* Monitor for signs of rebound congestion with prolonged use.
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions or if systemic symptoms are suspected.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to minimize the risk of rhinitis medicamentosa.
* Prime the pump mechanism before first use or if it has not been used for a while.
* Avoid contact with eyes.
* Use with caution in patients with heart disease, hypertension, diabetes, thyroid disease, or enlarged prostate.
