# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels, reducing swelling and congestion.
## Primary Indications
Symptomatic relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:**
* 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 applications in a 24-hour period.
* **Duration:** Do not use for more than 3 consecutive days.
## Pediatric Dosing
* **Children 6 to 12 years:**
* 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 applications in a 24-hour period.
* **Duration:** Do not use for more than 3 consecutive days.
* **Children under 6 years:** Safety and efficacy not established. Consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are generally required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing.
* **Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged or excessive use, hypertension, nervousness, insomnia, dizziness.
## Key Drug Interactions
* **MAOIs and other sympathomimetic agents:** May potentiate hypertensive crisis.
## Monitoring
* Monitor for signs of rebound congestion or nasal dryness with prolonged use.
* Assess for cardiovascular effects (e.g., elevated blood pressure) in susceptible individuals, although systemic absorption is minimal.
## Clinical Pearls
* Limit use to 3 consecutive days to prevent rebound congestion.
* If congestion persists, consider alternative therapies and reassess diagnosis.
* Avoid accidental eye contact.
* Store at room temperature.
