# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that acts as a topical decongestant. It causes vasoconstriction of the nasal mucosa, reducing swelling and improving airflow.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days. Rebound congestion (rhinitis medicamentosa) can occur with prolonged use.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1 to 2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Use is generally not recommended; consult a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients taking monoamine oxidase inhibitors (MAOIs) or within 2 weeks of discontinuing MAOIs.
* Narrow-angle glaucoma.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Less Common:** Rebound congestion (rhinitis medicamentosa), headache, nervousness, insomnia, palpitations, hypertension.
## Key Drug Interactions
* **MAOIs:** Risk of severe hypertensive crisis.
* **Beta-blockers:** May potentiate hypertensive effects.
* **Tricyclic antidepressants:** May potentiate hypertensive effects.
## Monitoring
* Assess for symptom relief.
* Monitor for signs of rebound congestion or systemic absorption (e.g., increased blood pressure, palpitations).
* Ensure duration of use does not exceed 3 days.
## Clinical Pearls
* Oxymetazoline provides symptomatic relief but does not treat the underlying cause of nasal congestion.
* Limit use to a maximum of 3 days to prevent rhinitis medicamentosa.
* If symptoms persist beyond 3 days, advise the patient to discontinue use and consult a healthcare professional.
* Avoid spraying directly onto the nasal septum.
