# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a direct-acting alpha-adrenergic agonist that acts as a topical decongestant. It causes vasoconstriction of blood vessels in the nasal mucosa, reducing swelling and congestion.
## Primary Indications
Symptomatic relief of nasal congestion due to the common cold, hay fever, or other upper respiratory allergies.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days to avoid rebound congestion (rhinitis medicamentosa).
## Pediatric Dosing
* **Children 6-12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Consult a pediatrician. Safety and efficacy not established.
## Dose Adjustments
No dose adjustment is typically required for renal or hepatic impairment, as systemic absorption is minimal with topical use.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients with narrow-angle glaucoma.
* Use with monoamine oxidase inhibitors (MAOIs) or within 2 weeks of discontinuing MAOIs (potential for hypertensive crisis).
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, dryness, runny nose.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic effects (hypertension, palpitations, headache, insomnia) especially with overuse or in susceptible individuals.
## Key Drug Interactions
* **MAOIs:** Concomitant use or use within 14 days of MAOI discontinuation can lead to severe hypertension.
* **Other sympathomimetics:** Potential for additive pressor effects.
## Monitoring
* Monitor for nasal symptoms and signs of rebound congestion.
* Monitor blood pressure and heart rate in patients with cardiovascular disease or if systemic absorption is suspected.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days.
* If symptoms persist beyond 3 days, advise patient to discontinue and consult a healthcare provider.
* Proper nasal spray technique is important for efficacy and to minimize systemic absorption.
* Use with caution in patients with cardiovascular disease, hypertension, hyperthyroidism, diabetes mellitus, or prostatic hyperplasia.
