# Sinex (oxymetazoline)
## Overview
Oxymetazoline is a topical alpha-adrenergic agonist that causes vasoconstriction of nasal mucosa, reducing swelling and congestion.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 0.05% solution.
* **Dose:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in 24 hours.
* **Duration:** Limit use to a maximum of 3 consecutive days.
## Pediatric Dosing
* **Nasal Spray (0.025% for children under 6):** Not typically recommended for children under 6 years of age. Consult a pediatrician for appropriate treatment.
* **Nasal Spray (0.05% for children 6-12 years):** Use with adult supervision.
* **Dose:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Maximum:** Do not exceed 2 doses in 24 hours.
* **Duration:** Limit use to a maximum of 3 consecutive days.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients using monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
## Adverse Effects
* **Common:** Nasal burning, stinging, dryness, sneezing, increased nasal discharge.
* **Rebound congestion (rhinitis medicamentosa):** Occurs with prolonged or excessive use (>3 days), leading to worsening congestion.
* **Systemic effects (rare but serious):** Increased blood pressure, nervousness, insomnia, palpitations, headache, dizziness.
## Key Drug Interactions
* **MAOIs and other sympathomimetic agents:** Increased risk of hypertensive crisis. Avoid concomitant use.
## Monitoring
* Monitor for signs of rebound congestion or overuse.
* Monitor blood pressure, especially in patients with pre-existing hypertension or cardiovascular disease.
## Clinical Pearls
* Limit use to a maximum of 3 consecutive days to prevent rebound congestion.
* If congestion persists after 3 days, discontinue oxymetazoline and consult a healthcare provider.
* Advise patients to prime the pump before first use and after storage.
* Avoid spraying into the eyes.
