# Sinex (oxymetazoline)
## Overview
Oxymetazoline is an alpha-adrenergic agonist that causes vasoconstriction of nasal mucosal blood vessels. This reduces swelling and congestion. It is available as a nasal spray.
## Primary Indications
* Nasal congestion associated with the common cold, allergic rhinitis, or sinusitis.
## Adult Dosing
* **Nasal Spray:** 2 sprays in each nostril every 10-12 hours as needed.
* **Maximum Duration:** Do not use for more than 3 consecutive days. Rebound congestion (rhinitis medicamentosa) can occur with prolonged use.
## Pediatric Dosing
* **Children 6 to under 12 years:** 1-2 sprays in each nostril every 10-12 hours as needed.
* **Children under 6 years:** Safety and efficacy not established. Consult pediatrician.
* **Maximum Duration:** Do not use for more than 3 consecutive days.
## Dose Adjustments
No specific dose adjustments are necessary for hepatic or renal impairment. However, caution is advised in patients with cardiovascular disease or uncontrolled hypertension.
## Contraindications
* Hypersensitivity to oxymetazoline or any component of the formulation.
* Patients taking monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOIs.
## Adverse Effects
* **Common:** Nasal discomfort, burning, stinging, sneezing, increased nasal discharge, dry mouth/throat, headache.
* **Less Common/Serious:** Rebound congestion (rhinitis medicamentosa) with prolonged use, systemic absorption leading to increased blood pressure, palpitations, tachycardia, insomnia, nervousness.
## Key Drug Interactions
* **MAOIs and other sympathomimetics:** Increased risk of hypertensive crisis. Avoid concurrent use.
* **Beta-blockers:** May potentiate pressor effect.
## Monitoring
* Monitor for signs of rebound congestion (worsening nasal congestion after medication wears off).
* Monitor blood pressure and heart rate in patients with pre-existing cardiovascular conditions.
## Clinical Pearls
* Limit use to 3 days to prevent rhinitis medicamentosa.
* Advise patients to blow their nose before use.
* Instruct patients to prime the pump if not used for a while.
* Keep the tip clean.
