# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a direct-acting sympathomimetic amine that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the arterioles in the nasal mucosa, resulting in decreased nasal congestion.
## Primary Indications
* Temporary relief of nasal congestion associated with the common cold, hay fever, or upper respiratory allergies.
* Sinus congestion and pressure.
## Adult Dosing
* **Standard Concentration:** 0.05% nasal spray.
* **Dose:** 2 to 3 sprays in each nostril every 10–12 hours.
* **Maximum:** Do not exceed 2 doses in any 24-hour period.
## Pediatric Dosing
* **Children 6 to <12 years:** 0.025% to 0.05% nasal spray. 2 to 3 sprays in each nostril every 10–12 hours. Do not exceed 2 doses in 24 hours.
* **Children <6 years:** Generally not recommended; consult a pediatrician for specific local protocols regarding lower-concentration formulations.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific guidelines available; use with caution.
* **Elderly:** Use with caution due to potential for systemic absorption resulting in increased blood pressure or heart rate.
## Contraindications
* Hypersensitivity to oxymetazoline or any components of the specific product.
* Use of monoamine oxidase inhibitors (MAOIs) currently or within the past 14 days (risk of hypertensive crisis).
## Adverse Effects
* **Local:** Rebound congestion (*rhinitis medicamentosa*), stinging, burning, sneezing, or increased nasal discharge.
* **Systemic (rare, if absorbed):** Hypertension, palpitations, tachycardia, insomnia, headache, and nervousness.
## Key Drug Interactions
* **MAOIs:** May cause severe hypertensive crisis; avoid concurrent use.
* **Tricyclic Antidepressants (TCAs) / Maprotiline:** May potentiate the pressor effects of oxymetazoline.
* **Beta-blockers:** May oppose the effects or potentially worsen hypertension through unopposed alpha-stimulation if systemic absorption occurs.
## Monitoring
* Monitor for duration of use; limit therapy to **3 consecutive days**.
* Monitor for signs of systemic sympathomimetic toxicity (tachycardia, elevated blood pressure).
## Clinical Pearls
* **Rhinitis Medicamentosa:** This is the most critical safety concern. Chronic use (>3 days) leads to tolerance and debilitating rebound congestion upon cessation.
* **Administration:** Instruct patients to blow their nose before use and to clean the nozzle after each application to prevent contamination.
* **Systemic Absorption:** Although localized, systemic absorption can occur, especially in patients with mucosal trauma or excessive dosing.
