# Sinex (Oxymetazoline)
## Overview
Oxymetazoline is a direct-acting sympathomimetic amine that acts as an alpha-adrenergic agonist. It causes vasoconstriction of the arterioles in the nasal mucosa, providing temporary relief of nasal congestion.
## Primary Indications
* Nasal congestion (common cold, sinusitis, allergies).
## Adult Dosing
* **Intranasal:** 2–3 sprays (0.05%) in each nostril every 10–12 hours.
* **Maximum Dose:** Do not exceed 2 doses in any 24-hour period.
## Pediatric Dosing
* **Children 6 to 12 years:** 2–3 sprays (0.025% formulation) in each nostril every 10–12 hours.
* **Children < 6 years:** Not recommended.
* **Maximum Dose:** Do not exceed 2 doses in any 24-hour period.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific adjustments provided by manufacturer labeling; however, use with caution due to potential systemic absorption.
## Contraindications
* Hypersensitivity to oxymetazoline.
* Use within 14 days of MAO inhibitor therapy.
* Concurrent use of other sympathomimetic decongestants.
## Adverse Effects
* **Common:** Nasal stinging, burning, dryness, sneezing, or increased rhinorrhea.
* **Serious (Systemic):** Hypertension, tachycardia, palpitations, tremor, insomnia, and rebound congestion (*rhinitis medicamentosa*).
## Key Drug Interactions
* **MAO Inhibitors:** Risk of hypertensive crisis.
* **Beta-Blockers/Antihypertensives:** Potential for reduced effectiveness of blood pressure control or increased risk of cardiovascular instability.
* **Tricyclic Antidepressants:** May potentiate the pressor effects of oxymetazoline.
## Monitoring
* Monitor for duration of use (limit to 3 consecutive days to prevent rebound congestion).
* Assess blood pressure and heart rate if the patient has underlying cardiovascular disease.
## Clinical Pearls
* **Rhinitis Medicamentosa:** Absolutely limit use to 3 consecutive days. Prolonged use causes tissue edema, resulting in a cycle of worsening congestion and increased drug usage.
* **Consultation:** If symptoms persist beyond 3 days, advise the patient to discontinue use and consult a physician.
* **Product Clarity:** Ensure the patient selects the appropriate concentration for their age (0.025% vs 0.05%).
* **Caution:** Exercise extreme caution in patients with uncontrolled hypertension, hyperthyroidism, diabetes, or prostatic hypertrophy.
