# Piperacillin/Tazobactam
## Overview
Piperacillin/tazobactam is an extended-spectrum penicillin antibiotic combined with a beta-lactamase inhibitor. It is active against many Gram-positive, Gram-negative, and anaerobic bacteria.
## Primary Indications
* Complicated intra-abdominal infections
* Complicated skin and skin structure infections
* Community-acquired pneumonia
* Nosocomial pneumonia
* Febrile neutropenia (empiric treatment)
## Adult Dosing
* **Standard Dosing:** 3.375 grams (3 grams piperacillin/0.375 grams tazobactam) intravenously every 6 hours.
* **Higher Dosing (for severe infections or those with reduced susceptibility):** 4.5 grams (4 grams piperacillin/0.5 grams tazobactam) intravenously every 6 or 8 hours.
* **Maximum Recommended Dose:** 18 grams per day (16 grams piperacillin/2 grams tazobactam).
* **Infusion:** Typically infused over 30 minutes. Longer infusions (e.g., 4 hours) may be considered for critical illness to achieve higher trough concentrations.
Dosing frequency and duration depend on the indication, severity of infection, and clinical response. Specific dosing for indications like febrile neutropenia may vary by institutional protocol.
## Pediatric Dosing
* **Children < 12 years of age:** Dosing is typically based on piperacillin component:
* **300-400 mg/kg/day of piperacillin**, divided into doses every 6-8 hours.
* The weight-based dose of tazobactam should be 1/8th of the piperacillin dose.
* Maximum daily dose of piperacillin should not exceed 18 grams/day.
* **Children ≥ 12 years of age:** Generally dosed as adults.
Dosing in neonates and infants requires careful consideration of renal and hepatic function and may vary by institutional protocol.
## Dose Adjustments
* **Renal Impairment:**
* CrCl ≥ 40 mL/min: No dose adjustment needed.
* CrCl 20-39 mL/min: Administer usual dose every 8 hours.
* CrCl < 20 mL/min: Administer usual dose every 12 hours.
* For higher doses (e.g., 4.5g q6h), adjustments will differ. Consult prescribing information.
* Hemodialysis: Administer usual dose every 12 hours and supplement with an additional dose (3.375g or 4.5g depending on dose) after each dialysis session.

## Contraindications
* Hypersensitivity to piperacillin, tazobactam, other penicillins, or beta-lactamase inhibitors.
* History of allergic reactions to cephalosporins or other beta-lactam antibiotics.
## Adverse Effects
* **Common:** Diarrhea, rash, nausea, vomiting, headache, insomnia, constipation, pruritus, elevated liver enzymes.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions (including anaphylaxis), seizures (particularly with high doses or renal impairment), neutropenia, thrombocytopenia.
## Key Drug Interactions
* **Probenecid:** May increase and prolong piperacillin/tazobactam serum concentrations.
* **Neuromuscular Blocking Agents (e.g., vecuronium):** Piperacillin may prolong the neuromuscular blockade.
* **Warfarin:** May decrease the effect of warfarin; increased INR monitoring may be necessary.
* **Methotrexate:** Piperacillin may decrease methotrexate clearance; monitor methotrexate levels.
* **Tetracyclines, Chloramphenicol:** Potential antagonism.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for diarrhea and assess for *C. difficile*.
* Monitor renal function, particularly in patients with underlying renal disease or receiving concomitant nephrotoxic agents.
* Monitor liver function tests.
* Monitor complete blood count, especially with prolonged therapy, for hematologic changes.
* Monitor electrolytes, especially during prolonged or high-dose therapy.
## Clinical Pearls
* Piperacillin/tazobactam covers Pseudomonas aeruginosa.
* Tazobactam extends the spectrum of piperacillin to include many beta-lactamase producing bacteria.
* Reconstituted solutions are typically stable for 24 hours at room temperature or 48 hours when refrigerated.
* The 3.375g formulation contains 0.375g tazobactam and 3g piperacillin. The 4.5g formulation contains 0.5g tazobactam and 4g piperacillin.
