# Piperacillin/Tazobactam
## Overview
Piperacillin/tazobactam is a combination antibiotic consisting of piperacillin, a broad-spectrum penicillin antibiotic, and tazobactam, a beta-lactamase inhibitor. It is active against many Gram-positive, Gram-negative, and anaerobic bacteria.
## Primary Indications
* Complicated intra-abdominal infections
* Complicated skin and skin-structure infections
* Community-acquired pneumonia
* Hospital-acquired pneumonia (including ventilator-associated pneumonia)
## Adult Dosing
* **Standard Dose:** 3.375 grams (3g piperacillin/0.375g tazobactam) or 4.5 grams (4g piperacillin/0.5g tazobactam) intravenously every 6 to 8 hours.
* **Dosing Frequency:** Typically every 6 hours for most indications. Every 8 hours may be used for less severe infections or in combination regimens.
* **Duration:** Varies based on indication and patient response, typically 4 to 14 days.
* **Higher Doses:** Doses up to 4.5 grams IV every 4 hours may be considered for severe infections or those with suspected or documented P. aeruginosa.
## Pediatric Dosing
* **Patients 0 to 12 months:** Dosing is weight-based and depends on the indication and severity of infection. Specific protocols should be followed.
* **Patients 1 to 12 years:**
* **Dosing:** 80 mg piperacillin/10 mg tazobactam per kg per dose intravenously every 8 hours.
* **Maximum Dose:** 3.375 grams per dose.
* **For P. aeruginosa:** 100 mg piperacillin/12.5 mg tazobactam per kg per dose intravenously every 8 hours, not to exceed 4.5 grams per dose.
* **Duration:** Varies based on indication and patient response.
## Dose Adjustments
* **Renal Impairment:**
* CrCl > 40 mL/min: No dose adjustment needed.
* CrCl 20-40 mL/min: Reduce dose by 25% or increase interval to every 8 hours.
* CrCl < 20 mL/min: Reduce dose by 50% or increase interval to every 12 hours.
* Hemodialysis: Administer dose after dialysis and increase interval to every 12 hours. Additional dose may be needed during dialysis.
* **Hepatic Impairment:** No specific dose adjustment is typically recommended, but caution may be advised.
## Contraindications
* Hypersensitivity to piperacillin, tazobactam, other penicillin antibiotics, or any component of the formulation.
* History of immediate or severe allergic reaction to beta-lactam antibiotics.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, rash, headache, insomnia, constipation, abdominal pain, fever.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions (anaphylaxis), seizures (especially with high doses or renal impairment), liver enzyme elevations, interstitial nephritis, hematologic changes (neutropenia, thrombocytopenia, eosinophilia).
## Key Drug Interactions
* **Probenecid:** May increase and prolong piperacillin/tazobactam serum concentrations.
* **Aminoglycosides:** Concurrent use may result in additive or synergistic activity, but physical incompatibility exists. Do not mix in the same IV line without considering appropriate dilution and flushing.
* **Vecuronium:** Piperacillin may prolong the neuromuscular blockade of vecuronium.
* **Warfarin:** May alter the anticoagulant response in patients receiving warfarin. Monitor INR closely.
* **Methotrexate:** Piperacillin may decrease methotrexate clearance. Monitor methotrexate levels.
## Monitoring
* Renal function (especially with impaired renal function or concurrent nephrotoxic agents).
* Liver function tests.
* Complete blood count (CBC) with differential, especially with prolonged therapy.
* Signs and symptoms of hypersensitivity reactions.
* Signs and symptoms of *C. difficile* infection.
* INR if patient is on warfarin.
* Methotrexate levels if applicable.
## Clinical Pearls
* Piperacillin/tazobactam is often reconstituted with sterile water for injection and then further diluted in an appropriate IV fluid (e.g., 0.9% sodium chloride). Refer to manufacturer's instructions for preparation.
* Infusion time is typically 30 minutes. Longer infusion times may be considered for higher doses or to achieve higher Cmax.
* The beta-lactamase inhibitor tazobactam broadens the spectrum of piperacillin to include many beta-lactamase producing organisms.
* Consider patient's renal function for appropriate dose adjustments.
