# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that reduces stomach acid production.
## Primary Indications
* Gastroesophageal reflux disease (GERD)
* Erosive esophagitis
* Zollinger-Ellison syndrome
* Healing of duodenal ulcers
* H. pylori eradication (in combination therapy)
## Adult Dosing
* **GERD and Erosive Esophagitis:** 20 mg to 40 mg orally or intravenously once daily for 4 to 8 weeks.
* **Maintenance:** 20 mg orally once daily.
* **Zollinger-Ellison Syndrome:** 40 mg orally or intravenously once daily, may be increased to 80 mg intravenously or orally every 8 hours.
* **Duodenal Ulcers:** 40 mg orally once daily for up to 4 weeks.
* **H. pylori Eradication:** Part of a multi-drug regimen, typically 40 mg orally twice daily for 7-14 days.
## Pediatric Dosing
* **GERD (12 years and older):** 20 mg orally once daily for up to 8 weeks.
* **Erosive Esophagitis (5 years and older):** 20 mg to 40 mg orally once daily for up to 8 weeks.
* **Note:** Dosing for younger pediatric patients and for other indications is less established and may require specialist consultation.
## Dose Adjustments
* **Hepatic Impairment:** Maximum dose of 20 mg orally or 40 mg intravenously once daily.
## Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
* **Common:** Headache, diarrhea, nausea, abdominal pain, flatulence.
* **Serious:** *Clostridium difficile*-associated diarrhea, bone fractures (hip, wrist, spine) with long-term use, hypomagnesemia, vitamin B12 deficiency, lupus erythematosus.
## Key Drug Interactions
* **Drugs dependent on gastric pH for absorption:** Increased absorption of drugs like ketoconazole, itraconazole, posaconazole, and potentially others.
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity. Monitor closely.
* **CYP2C19 substrates:** Pantoprazole is a moderate inhibitor of CYP2C19.
## Monitoring
* Electrolyte levels (magnesium, sodium, potassium) with prolonged therapy.
* Bone mineral density with long-term use.
* Signs and symptoms of *C. difficile* infection.
## Clinical Pearls
* Administer oral pantoprazole 30-60 minutes before a meal.
* Intravenous administration requires reconstitution and dilution.
* Long-term PPI use is associated with increased risks of fractures, *C. difficile*, and nutrient deficiencies. Consider shortest effective duration.
