# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
## Primary Indications
* Healing of erosive esophagitis associated with GERD.
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions (e.g., Zollinger-Ellison syndrome).
## Adult Dosing
* **Erosive esophagitis:** 40 mg orally or intravenously once daily for up to 8 weeks.
* **Maintenance of healing:** 40 mg orally once daily. Some patients may be managed on 20 mg orally once daily.
* **Pathological hypersecretory conditions:** Starting dose is typically 40 mg orally or intravenously twice daily. Doses may be titrated up to 240 mg daily based on response.
## Pediatric Dosing
* **Erosive esophagitis (12 years and older):** 40 mg orally once daily for up to 8 weeks.
* **Erosive esophagitis (5 to 11 years):** 20 mg orally once daily for up to 8 weeks.
* **Erosive esophagitis (1 month to <5 years):** 1.1 mg/kg/day orally or intravenously once daily, not to exceed 40 mg/day. Duration of therapy should be individualized.

## Dose Adjustments
No dose adjustment is generally required for renal impairment or hepatic impairment, although caution may be advised in severe hepatic impairment.
## Contraindications
Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, dizziness, and flatulence. Long-term use may be associated with an increased risk of fractures (hip, wrist, spine), Clostridium difficile infection, and vitamin B12 deficiency.
## Key Drug Interactions
* **Drugs dependent on gastric pH for absorption:** Ketoconazole, itraconazole, posaconazole, erlotinib, mycophenolate mofetil, nelfinavir, rilpivirine.
* **Methotrexate:** PPIs may increase methotrexate levels.
* **CYP2C19 substrates:** Monitor for potential interactions with drugs like clopidogrel, though clinical significance can vary.
## Monitoring
* Monitor for improvement in GERD symptoms.
* Assess for adverse effects, particularly with long-term use.
* Consider vitamin B12 levels with prolonged therapy (>3 years).
## Clinical Pearls
* Administer oral pantoprazole 30 minutes before a meal.
* Intravenous administration requires reconstitution and dilution; follow manufacturer instructions.
* Do not crush or chew delayed-release tablets.
* Consider using the lowest effective dose for maintenance therapy.
