# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
## Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
## Adult Dosing
* **Healing of Erosive Esophagitis:** 40 mg orally once daily for up to 8 weeks.
* **Maintenance of Healing of Erosive Esophagitis:** 40 mg orally once daily. Some patients may be treated with 20 mg orally once daily.
* **Pathological Hypersecretory Conditions (e.g., Zollinger-Ellison Syndrome):** Doses range from 40 mg orally once daily up to a maximum of 80 mg intravenously every 8 hours or 40 mg intravenously twice daily. Individualized dosing is required.
## Pediatric Dosing
* **Age 5 years and older:**
* **Erosive Esophagitis:** 40 mg orally once daily for up to 8 weeks.
* **GERD Symptomatic Treatment:** 20 mg orally once daily for up to 8 weeks.
* Dosing for younger pediatric patients is not well established.
## Dose Adjustments
* **Hepatic Impairment:** Maximum dose of 40 mg orally once daily.
* **Renal Impairment:** No dose adjustment necessary for oral formulations.
## Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
* **Common:** Headache, diarrhea, nausea, abdominal pain, flatulence, vomiting, dizziness, arthralgia.
* **Serious:** *Clostridioides difficile*-associated diarrhea, bone fractures (hip, wrist, spine) with long-term use, hypomagnesemia (can be severe, leading to tetany, seizures, arrhythmias), vitamin B12 deficiency, fundic gland polyps.
## Key Drug Interactions
* **Drugs Dependent on Gastric pH for Absorption:** Ketoconazole, itraconazole, posaconazole, iron salts, cyanocobalamin, mycophenolate mofetil. Pantoprazole may decrease absorption.
* **Methotrexate:** PPIs may increase methotrexate levels, potentially leading to toxicity.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19. Monitor for increased concentrations of drugs like clopidogrel.
## Monitoring
* Monitor for signs and symptoms of *Clostridioides difficile*-associated diarrhea.
* Consider monitoring serum magnesium levels in patients on long-term therapy (typically >1 year) or those taking concomitant medications known to lower magnesium (e.g., diuretics).
* Monitor for signs of bone fracture.
* Monitor for vitamin B12 deficiency symptoms with prolonged use.
## Clinical Pearls
* Administer pantoprazole orally 30-60 minutes before a meal.
* For intravenous administration, reconstitute and dilute according to manufacturer instructions.
* Long-term use of PPIs is associated with an increased risk of certain adverse events. Consider the lowest effective dose for the shortest duration needed.
* Parenteral formulations are generally reserved for patients unable to take oral medications.
