## Pantoprazole
### Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
### Primary Indications
* Gastroesophageal reflux disease (GERD)
* Erosive esophagitis
* Healing of duodenal ulcers
* Treatment of *Helicobacter pylori* infection (in combination regimens)
* Prophylaxis of NSAID-induced gastric ulcers
### Adult Dosing
* **GERD/Erosive Esophagitis:** 40 mg orally or intravenously once daily. Maximum 40 mg daily for erosive esophagitis.
* **Duodenal Ulcer Healing:** 40 mg orally once daily.
* ***H. pylori* Eradication:** 40 mg orally twice daily in combination with antibiotics for 7-14 days.
* **NSAID Ulcer Prophylaxis:** 20 mg orally once daily.
* **IV Administration:** 40 mg intravenously once daily.
### Pediatric Dosing
* **GERD/Erosive Esophagitis (12 years and older):** 40 mg orally once daily.
* **GERD (5 years to 11 years):**
* **Healing of Erosive Esophagitis:** 20 mg orally once daily for up to 8 weeks.
* **Maintenance of Healing:** 10-20 mg orally once daily.
* **Symptomatic GERD:** 15 mg orally once daily.
* *Dosing in younger children is not well established and may depend on local protocols.*
### Dose Adjustments
* **Hepatic Impairment:** Maximum daily dose of 20 mg orally.
* **Renal Impairment:** No dose adjustment necessary.
### Contraindications
* Hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
* Common: Diarrhea, headache, nausea, abdominal pain, flatulence.
* Long-term use: Increased risk of *Clostridium difficile*-associated diarrhea, bone fractures (hip, wrist, spine), hypomagnesemia, vitamin B12 deficiency.
### Key Drug Interactions
* **Drugs requiring gastric acid for absorption:** Ketoconazole, itraconazole, nelfinavir, rilpivirine (monitor for decreased efficacy).
* **Methotrexate:** May increase and prolong serum levels of methotrexate, potentially leading to toxicity.
* **CYP2C19 substrates (e.g., clopidogrel):** Potential for reduced efficacy of clopidogrel due to CYP2C19 inhibition.
### Monitoring
* Magnesium levels (especially with prolonged use or concurrent diuretic use).
* Vitamin B12 levels (with prolonged therapy).
* Signs and symptoms of *C. difficile* infection.
* Bone mineral density (consider for patients at risk for osteoporosis).
### Clinical Pearls
* Administer oral pantoprazole 30 minutes before a meal.
* IV pantoprazole should be infused over 2-5 minutes or as an infusion over 15 minutes.
* Long-term PPI use is associated with potential risks; consider the shortest effective duration.
* The risk of fracture increases with higher doses and longer duration of therapy.
