## Pantoprazole
### Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
### Primary Indications
* Healing of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
### Adult Dosing
* **Erosive esophagitis:** 40 mg orally or intravenously once daily for up to 8 weeks.
* **Maintenance of healing:** 40 mg orally once daily. Some patients may be treated with 20 mg orally once daily.
* **Pathological hypersecretory conditions:** Starting dose is 40 mg orally or intravenously twice daily. Doses can be increased as needed. Doses up to 240 mg daily have been administered intravenously.
### Pediatric Dosing
* **1-5 years (weighing 15-40 kg):** 20 mg orally once daily.
* **6-16 years (weighing >40 kg):** 40 mg orally once daily.
* **GERD (erosive esophagitis):** Dosing varies based on age and weight, typically 20-40 mg orally once daily for up to 8 weeks. Specific dosing recommendations should be followed based on current guidelines.
### Dose Adjustments
No dose adjustment is typically required for hepatic or renal impairment.
### Contraindications
* Known hypersensitivity to pantoprazole or any component of the formulation.
### Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, and dizziness. Long-term use of PPIs may be associated with an increased risk of *Clostridioides difficile* infection, bone fractures, vitamin B12 deficiency, and hypomagnesemia.
### Key Drug Interactions
* **Antiretrovirals (e.g., nelfinavir, rilpivirine):** Pantoprazole may decrease absorption and efficacy.
* **Methotrexate:** PPIs may increase methotrexate levels.
* **CYP2C19 substrates (e.g., clopidogrel):** Pantoprazole may affect the metabolism of certain drugs metabolized by CYP2C19. While pantoprazole has a weaker interaction with clopidogrel compared to omeprazole or esomeprazole, caution is still advised.
### Monitoring
Monitor for signs and symptoms of *Clostridioides difficile* infection, bone fractures, and electrolyte abnormalities (especially magnesium).
### Clinical Pearls
* Administer oral pantoprazole 30 minutes to 1 hour before a meal.
* Intravenous administration requires reconstitution and dilution.
