# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
## Primary Indications
* Healing of erosive esophagitis
* Gastroesophageal reflux disease (GERD)
* Zollinger-Ellison syndrome
* Part of H. pylori eradication regimens
## Adult Dosing
* **Erosive esophagitis:** 40 mg orally once daily for 4-8 weeks. Maintenance therapy may be 20 mg orally once daily.
* **GERD:** 20-40 mg orally once daily.
* **Zollinger-Ellison syndrome:** Starting dose typically 40 mg orally twice daily, may be increased to 240 mg daily in divided doses. Max dose: 240 mg daily.
* **H. pylori eradication:** Typically given as 40 mg orally twice daily in combination with antibiotics.
## Pediatric Dosing
* **Erosive esophagitis (12 years and older):** 40 mg orally once daily for 4-8 weeks.
* **GERD (5 years and older):** 20-40 mg orally once daily.
* Dosing for younger children is not well-established and requires careful consideration based on indication and weight.
## Dose Adjustments
No dose adjustment needed for hepatic or renal impairment.
## Contraindications
* Known hypersensitivity to pantoprazole or other PPIs.
## Adverse Effects
* Common: Headache, diarrhea, nausea, abdominal pain, dizziness.
* Long-term use: Increased risk of fractures (hip, wrist, spine), vitamin B12 deficiency, hypomagnesemia, C. difficile-associated diarrhea, and potentially certain gastric cancers.
## Key Drug Interactions
* **Antiretrovirals:** May decrease absorption of drugs like atazanavir and nelfinavir.
* **Methotrexate:** PPIs may increase methotrexate levels.
* **Clopidogrel:** Data is conflicting, but some studies suggest a potential decrease in clopidogrel's antiplatelet effect.
## Monitoring
* Monitor for symptoms of hypomagnesemia (e.g., muscle spasms, seizures, tremors) if used long-term.
* Consider vitamin B12 levels with prolonged therapy.
* Monitor for C. difficile infection.
## Clinical Pearls
* Administer orally 30-60 minutes before a meal.
* If using the orally disintegrating tablet (ODT), do not chew or crush; place on the tongue and swallow.
* Consider lowest effective dose for maintenance therapy to minimize risks associated with long-term use.
