# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that reduces gastric acid production.
## Primary Indications
* Healing of erosive esophagitis associated with GERD
* Maintenance of healing of erosive esophagitis
* Treatment of pathological hypersecretory conditions (e.g., Zollinger-Ellison syndrome)
## Adult Dosing
* **Healing of erosive esophagitis:** 40 mg orally once daily for up to 8 weeks.
* **Maintenance of healing of erosive esophagitis:** 40 mg orally once daily.
* **Pathological hypersecretory conditions:** Starting dose is typically 40 mg orally twice daily. Doses may be increased as clinically indicated. Usual doses range from 40 mg to 80 mg daily. Maximum dose generally not to exceed 80 mg IV or 40 mg orally twice daily.
## Pediatric Dosing
Dosing varies by indication and age. Consult specific pediatric guidelines.
* **Erosive esophagitis (12 to 17 years):** 40 mg orally once daily for up to 4 weeks.
* **Pathological hypersecretory conditions (adults and pediatric patients 5 years and older):** Dosing varies. IV dosing has been studied in specific populations.
## Dose Adjustments
* **Hepatic Impairment:** Maximum daily dose of 40 mg orally or 40 mg IV.
* **Renal Impairment:** No dose adjustment generally recommended.
## Contraindications
* Hypersensitivity to pantoprazole or any component of the formulation.
## Adverse Effects
Common adverse effects include headache, diarrhea, nausea, abdominal pain, and dizziness. Long-term PPI use may be associated with an increased risk of bone fractures, Clostridium difficile infection, and vitamin B12 deficiency.
## Key Drug Interactions
* **Certain Antiretrovirals (e.g., rilpivirine, atazanavir):** Pantoprazole can decrease absorption of these drugs. Avoid coadministration or monitor closely.
* **Methotrexate:** PPIs may increase methotrexate levels. Consider temporarily discontinuing pantoprazole during high-dose methotrexate administration.
* **CYP2C19 Substrates:** Pantoprazole is a moderate inhibitor of CYP2C19. Monitor for increased levels of drugs metabolized by this enzyme.
## Monitoring
* Assess for improvement of GERD symptoms.
* Monitor for adverse effects, especially with long-term use.
* Monitor electrolytes if edema occurs.
## Clinical Pearls
* Pantoprazole is often dosed once daily for GERD indications.
* For hypersecretory conditions, divided dosing may be required.
* Administer 30 minutes before a meal.
* IV administration is an alternative when oral route is not feasible.
