### Pantoprazole
**Overview**
Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion.
**Primary Indications**
* Gastroesophageal reflux disease (GERD)
* Healing of erosive esophagitis
* Zollinger-Ellison syndrome
* H. pylori eradication (in combination with antibiotics)
**Adult Dosing**
* **GERD/Erosive Esophagitis:** 40 mg orally or intravenously once daily. Duration of therapy varies based on indication.
* **Zollinger-Ellison Syndrome:** Starting dose is typically 40 mg orally or intravenously twice daily, may be titrated up to 240 mg daily in divided doses.
* **H. pylori Eradication:** 40 mg orally twice daily in combination with specific antibiotics (e.g., amoxicillin and clarithromycin or clarithromycin and metronidazole) for 7-14 days.
**Pediatric Dosing**
* **GERD (age ≥ 5 years):**
* Healing of erosive esophagitis: 40 mg orally once daily.
* Symptomatic GERD: 20 mg orally once daily. Dosing may be adjusted based on clinical response.
* *There is limited data for pantoprazole in pediatric patients younger than 5 years.*
**Dose Adjustments**
* **Hepatic Impairment:** Maximum daily dose of 40 mg orally or 20 mg intravenously.
* **Renal Impairment:** No dose adjustment is generally recommended.
**Contraindications**
* Known hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
**Adverse Effects**
* **Common:** Headache, diarrhea, nausea, abdominal pain, flatulence, dizziness.
* **Serious:** *Clostridioides difficile*-associated diarrhea, bone fractures (hip, wrist, spine) with long-term, high-dose use, hypomagnesemia, vitamin B12 deficiency, fundic gland polyps.
**Key Drug Interactions**
* **Warfarin:** Increased INR and bleeding risk; monitor INR closely.
* **Methotrexate:** May increase methotrexate levels; consider temporary discontinuation of pantoprazole.
* **Drugs dependent on gastric pH for absorption (e.g., ketoconazole, itraconazole, iron salts, mycophenolate mofetil, rilpivirine):** May decrease absorption.
* **CYP2C19 substrates (e.g., clopidogrel):** May reduce the efficacy of clopidogrel.
**Monitoring**
* Monitor for signs and symptoms of *C. difficile*-associated diarrhea.
* Consider serum magnesium levels, especially with prolonged use (>1 year) or concurrent diuretic use.
* Monitor bone mineral density in patients at risk for osteoporosis.
* Monitor for signs of vitamin B12 deficiency with long-term use.
**Clinical Pearls**
* For H. pylori eradication, ensure adherence to the full antibiotic course.
* Long-term PPI use is associated with increased risks of fractures, C. difficile infection, hypomagnesemia, and vitamin B12 deficiency. Discuss risks and benefits with patients.
* Intravenous formulation is typically reserved for patients unable to take oral medication.
