# Pantoprazole
## Overview
Pantoprazole is a substituted benzimidazole proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H+/K+-ATPase enzyme system at the secretory surface of the gastric parietal cell. It is available as oral delayed-release tablets, granules for oral suspension, and intravenous (IV) injection.
## Primary Indications
* Erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Pathological hypersecretory conditions, including Zollinger-Ellison syndrome.
* Short-term treatment of GERD (off-label/guideline use).
## Adult Dosing
* **Erosive Esophagitis:** 40 mg PO/IV once daily for up to 8 weeks.
* **Maintenance of Healing:** 40 mg PO once daily.
* **Pathological Hypersecretory Conditions:** 40 mg PO twice daily; dosages up to 240 mg/day have been utilized; titration based on clinical response.
* **IV Administration:** Administer IV doses via rapid injection (2 minutes) or short infusion (15 minutes).
## Pediatric Dosing
* **GERD/Erosive Esophagitis (≥5 years):**
* 15 kg to <40 kg: 20 mg once daily.
* ≥40 kg: 40 mg once daily.
* Maximum duration: 8 weeks.
* **Neonates/Infants (<5 years):** Safety and efficacy not established for GERD; use is generally off-label and requires institutional protocol consultation.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment necessary.
* **Hepatic Impairment:** Use with caution; monitor liver function. Consider lower doses or less frequent administration in severe impairment.
## Contraindications
* Hypersensitivity to pantoprazole or any component of the formulation.
* Concomitant use with rilpivirine-containing products (due to decreased rilpivirine absorption).
## Adverse Effects
* **Common:** Headache, diarrhea, nausea, abdominal pain, flatulence.
* **Serious:** *Clostridioides difficile*-associated diarrhea (CDAD), hypomagnesemia, vitamin B12 deficiency (with long-term use), acute interstitial nephritis, and increased risk of bone fractures (long-term/high-dose).
## Key Drug Interactions
* **pH-Dependent Drugs:** PPIs decrease the absorption of drugs requiring acidic environments (e.g., atazanavir, ketoconazole, iron salts, mycophenolate mofetil).
* **CYP2C19 Inhibitors/Inducers:** May affect pantoprazole systemic exposure (though pantoprazole is generally considered to have fewer DDI risks than omeprazole).
* **Methotrexate:** Potential increase in methotrexate levels when used concurrently; consider holding PPI therapy during methotrexate administration.
## Monitoring
* **Baseline/Intermittent:** Magnesium levels (if long-term therapy is planned), Vitamin B12 (if long-term therapy is planned).
* **Clinical:** Monitor for signs of *C. difficile* (persistent/watery stools), bone density (in high-risk patients), and renal function.
## Clinical Pearls
* **Administration:** Tablets should be swallowed whole; do not crush or chew. Granules for oral suspension can be mixed with applesauce or juice and administered via nasogastric tube.
* **Tapering:** Long-term users may experience rebound hypersecretion upon discontinuation; consider a gradual taper for patients on long-term daily therapy.
* **"As Needed" Use:** PPIs are most effective when taken daily; "as needed" (PRN) dosing is generally ineffective for GERD management.
