# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that inhibits the H+/K+-ATPase enzyme system in the gastric parietal cell, suppressing both basal and stimulated gastric acid secretion. Standard formulations include delayed-release tablets and IV powder for reconstitution.
## Primary Indications
* Erosive esophagitis associated with GERD.
* Maintenance of healing of erosive esophagitis.
* Pathological hypersecretory conditions (e.g., Zollinger-Ellison syndrome).
* Prophylaxis of stress-induced upper GI bleeding (clinical practice, though often off-label).
## Adult Dosing
* **GERD/Erosive Esophagitis:** 40 mg PO or IV daily for 8 weeks; may extend another 8 weeks if necessary.
* **Maintenance of Healing:** 40 mg PO daily.
* **Hypersecretory Conditions:** Starting dose 40 mg twice daily. Doses up to 240 mg/day have been used clinically; titrate based on acid output.
* **Stress Ulcer Prophylaxis:** Common protocol is 40 mg IV daily or twice daily (follow institution-specific guidelines).
## Pediatric Dosing
* **Erosive Esophagitis (≥5 years):**
* 15 kg to <40 kg: 20 mg PO daily for up to 8 weeks.
* ≥40 kg: 40 mg PO daily for up to 8 weeks.
* **Safety/Efficacy:** Use in patients <5 years is generally not recommended due to limited data.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment necessary.
* **Hepatic Impairment:** Use caution; may consider a maximum dose of 20 mg daily or every other day in severe impairment (Child-Pugh C).
## Contraindications
* Known hypersensitivity to pantoprazole or any substituted benzimidazole (e.g., omeprazole, lansoprazole).
* Hypersensitivity to any component of the formulation.
## Adverse Effects
* **Common:** Headache, diarrhea, nausea, abdominal pain.
* **Serious:** *Clostridioides difficile*-associated diarrhea (CDAD), hypomagnesemia, Vitamin B12 deficiency (with long-term use), acute interstitial nephritis, and increased risk of bone fractures (hip, wrist, spine) with high-dose/long-term therapy.
## Key Drug Interactions
* **pH-Dependent Drugs:** May decrease absorption of drugs requiring acidic gastric pH (e.g., atazanavir, ketoconazole, iron salts, mycophenolate mofetil).
* **Methotrexate:** PPIs may increase serum levels of methotrexate, potentially leading to toxicity (monitor closely in high-dose therapy).
* **CYP2C19 Inhibitors/Inducers:** Minimal clinical impact compared to other PPIs, but monitor for altered efficacy.
## Monitoring
* **Long-term:** Serum magnesium (if on therapy >1 year), Vitamin B12 levels (if on therapy >2-3 years).
* **General:** Monitor for signs of GI bleeding, changes in bowel habits (*C. diff* risk), or new-onset unexplained bone pain.
## Clinical Pearls
* **Administration:** Tablets should be swallowed whole; do not split, crush, or chew.
* **Tapering:** Long-term users may experience rebound hyperacidity upon sudden discontinuation; consider a gradual taper.
* **IV vs Oral:** Transition to oral therapy as soon as patient can tolerate oral intake and the clinical condition is stable.
* **PPI Stewardship:** Review the necessity of continued PPI use periodically; avoid long-term use unless indicated for Barrett's esophagus, chronic NSAID use with risk factors, or hypersecretory conditions.
