# Pantoprazole
## Overview
Pantoprazole is a proton pump inhibitor (PPI) that inhibits the H+/K+-ATPase enzyme system in the gastric parietal cell, suppressing both basal and stimulated gastric acid secretion. It is available in oral (delayed-release tablets, granules for suspension) and intravenous (IV) formulations.
## Primary Indications
* Treatment of erosive esophagitis associated with gastroesophageal reflux disease (GERD).
* Maintenance of healing of erosive esophagitis.
* Hypersecretory conditions (e.g., Zollinger-Ellison syndrome).
* Short-term treatment of GERD-associated symptoms (off-label or indication-specific).
## Adult Dosing
* **Erosive Esophagitis:** 40 mg PO or IV once daily for up to 8 weeks.
* **Maintenance of Healing:** 40 mg PO once daily.
* **Zollinger-Ellison Syndrome:** Starting dose 40 mg PO twice daily. Doses up to 240 mg/day have been used based on clinical judgment.
* **GERD Symptoms (Short-term):** 20–40 mg PO once daily.
## Pediatric Dosing
* **GERD (Erosive Esophagitis, ages 5 years and older):**
* 15 kg to <40 kg: 20 mg once daily.
* ≥ 40 kg: 40 mg once daily.
* Safety and efficacy in patients younger than 5 years have not been established.
## Dose Adjustments
* **Hepatic Impairment:** Reduce frequency or total daily dose; monitor closely. No specific fixed dose adjustment formula is universally standardized; follow institutional protocol.
* **Renal Impairment:** No dose adjustment necessary.
## Contraindications
* Known hypersensitivity to pantoprazole or any substituted benzimidazole (e.g., omeprazole, lansoprazole).
* Patients receiving rilpivirine-containing products.
## Adverse Effects
* **Common:** Headache, diarrhea, nausea, abdominal pain, flatulence.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypomagnesemia, vitamin B12 deficiency (with long-term use), acute interstitial nephritis, cutaneous/systemic lupus erythematosus, and increased risk of bone fractures (long-term high-dose use).
## Key Drug Interactions
* **pH-Dependent Drugs:** May decrease absorption of drugs like atazanavir, nelfinavir, erlotinib, and some iron salts.
* **Clopidogrel:** Potential reduction in antiplatelet effect; clinical significance remains debated, but caution is advised.
* **Methotrexate:** PPIs may increase methotrexate blood levels, especially in high-dose therapy.
## Monitoring
* Monitor for signs of hypomagnesemia (during long-term therapy).
* Monitor renal function if symptoms of interstitial nephritis arise.
* Assess need for continued PPI therapy periodically; avoid unnecessary long-term use (deprescribing).
## Clinical Pearls
* **Administration:** Tablets should be swallowed whole; do not crush, split, or chew.
* **IV Administration:** IV pantoprazole should be administered as a 15-minute infusion or direct slow injection (2 minutes) to minimize site reactions.
* **Efficacy:** Acid suppression is more complete with repeated daily dosing; maximal benefit may take several days to achieve.
* **Proton Pump Inhibitor Selection:** Pantoprazole is frequently chosen in hospital settings due to its compatibility with many IV fluids and drugs, though it is considered slightly less potent at acid suppression than esomeprazole or rabeprazole.
