# Normal Saline
## Overview
Normal saline, also known as 0.9% sodium chloride solution, is an isotonic intravenous fluid used for hydration and electrolyte replacement.
## Primary Indications
* Volume resuscitation in hypovolemia and shock.
* Maintenance intravenous fluid therapy.
* Diluent for compatible intravenous medications.
* Treatment of hyponatremia (with caution).
## Adult Dosing
Dosing is highly individualized based on clinical status, degree of volume depletion, and underlying medical conditions.
* **Hypovolemia/Shock:** Loading doses of 1-2 liters intravenously rapidly are common. Further doses based on clinical response. Target goal is to restore hemodynamic parameters.
* **Maintenance:** Typically 1-2 liters intravenously over 24 hours, adjusted for insensible losses, urine output, and ongoing losses.
* **Medication Diluent:** Varies based on the specific medication and administration requirements.
## Pediatric Dosing
Dosing is highly individualized and should follow institutional protocols.
* **Hypovolemia/Shock:** Boluses of 10-20 mL/kg intravenously rapidly, repeated as needed based on clinical response.
* **Maintenance:** Typically 100 mL/kg/24 hours, adjusted for age, weight, and clinical condition.
## Dose Adjustments
* **Renal Impairment:** Use with caution. May lead to fluid overload and hyperchloremic acidosis.
* **Heart Failure/Renal Impairment:** Reduce rate of infusion to avoid fluid overload.
## Contraindications
* Known hypersensitivity to sodium chloride.
* Conditions where fluid overload is dangerous (e.g., severe heart failure, pulmonary edema).
* Hypernatremia.
## Adverse Effects
* **Fluid Overload:** Edema, hypertension, dyspnea, pulmonary congestion.
* **Hypernatremia:** Especially with rapid or excessive administration, particularly in patients with impaired water excretion.
* **Hyperchloremic Acidosis:** Can occur with large volume infusions due to the high chloride content.
* **Local Irritation/Phlebitis:** At the infusion site.
## Key Drug Interactions
None are anticipated with normal saline itself as it is a fluid. However, its use as a diluent can interact with the stability and compatibility of other drugs. Always check drug compatibility charts.
## Monitoring
* **Clinical Assessment:** Vital signs, mental status, urine output, signs of fluid overload (edema, crackles).
* **Electrolytes:** Sodium, chloride, bicarbonate, especially with large volumes or in patients with renal or cardiac issues.
* **Fluid Balance:** Intake and output.
## Clinical Pearls
* Normal saline is generally considered a safe and effective fluid for resuscitation and maintenance.
