# Normal Saline (0.9% Sodium Chloride)
## Overview
Normal Saline (NS) is an isotonic crystalloid solution containing 154 mEq/L each of sodium and chloride (osmolarity ~308 mOsm/L). It remains the most common intravenous maintenance and resuscitation fluid.
## Primary Indications
* Fluid resuscitation (hypovolemia, shock).
* Maintenance fluid therapy.
* Hyponatremia treatment (specifically hypovolemic hyponatremia).
* Vehicle for delivery of IV medications.
* Wound/irrigation and ophthalmic cleansing.
## Adult Dosing
* **Resuscitation:** Typically 500 mL to 1,000 mL bolus administered rapidly; repeat based on clinical response, hemodynamics, and signs of fluid overload.
* **Maintenance:** Generally 1–2 mL/kg/hour, or calculated via the Holliday-Segar method (100 mL/kg for first 10 kg, 50 mL/kg for next 10 kg, 20 mL/kg for remainder).
* *Note: Dosing is highly dependent on institutional protocols, patient clinical status, and underlying comorbidities.*
## Pediatric Dosing
* **Resuscitation:** 20 mL/kg bolus administered over 5–20 minutes. Repeat as clinically indicated (total volumes often monitored closely to avoid cerebral edema in specific clinical scenarios).
* **Maintenance:** Holliday-Segar method (100/50/20 rule):
* First 10 kg: 100 mL/kg/day.
* Next 11–20 kg: 50 mL/kg/day.
* Each kg > 20 kg: 20 mL/kg/day.

## Dose Adjustments
* **Renal/Hepatic Impairment:** Reduce volume and rate in patients with significant renal impairment or heart failure.

## Contraindications
* Hypernatremia.
* Fluid overload (e.g., congestive heart failure, severe renal failure with oliguria/anuria).
* Patients with documented hypersensitivity to sodium chloride solutions.
## Adverse Effects
* **Hyperchloremic Metabolic Acidosis:** High-volume administration of 0.9% NaCl can lead to non-anion gap metabolic acidosis due to chloride load.
* **Fluid Overload:** Peripheral/pulmonary edema, hypertension, and congestive heart failure exacerbation.
* **Electrolyte Imbalance:** Hypernatremia or dilutional hyponatremia/hypokalemia.
## Key Drug Interactions
* **Incompatibility:** Physically incompatible with certain medications; always consult drug compatibility charts (e.g., Trissel’s) prior to Y-site administration.
* **Corticosteroids/NSAIDS:** May increase risk of sodium and water retention.
## Monitoring
* **Hemodynamics:** Blood pressure, heart rate, capillary refill, and urine output.
* **Laboratory:** Serum electrolytes (Na, Cl, K), arterial blood gases (pH/bicarbonate), and renal function (BUN/Cr).
* **Clinical:** Frequent auscultation of lung sounds (crackles) and assessment for peripheral edema.
## Clinical Pearls
* **Balanced Crystalloids:** Consider balanced solutions (e.g., Lactated Ringer’s or Plasma-Lyte) over NS in patients at risk for hyperchloremic acidosis or those requiring large volumes, as NS can negatively impact renal blood flow.
* **Not Physiologic:** Despite the name "Normal," NS is not physiologically "normal"; it has supraphysiologic chloride concentrations (154 mEq/L vs. serum 98–107 mEq/L).
* **Dosing Limits:** There is no absolute maximum daily dose; administration is limited by the patient's capacity to tolerate volume expansion (i.e., signs of volume overload).
