## Overview
Mepivacaine is a local anesthetic of the amino-amide class. It blocks nerve impulse transmission by reversibly inhibiting sodium ion influx across the neuronal membrane.
## Primary Indications
* Local infiltration anesthesia
* Peripheral nerve blocks
* Epidural anesthesia
* Sympathetic blocks
## Adult Dosing
Dosing is highly variable and depends on the site, procedure, and patient factors. **Always consult local protocols or the manufacturer's package insert for specific dosing guidelines.**
* **Local infiltration:** Typically 5-300 mg.
* **Peripheral nerve blocks:** Typically 50-200 mg.
* **Epidural anesthesia:** Typically 100-200 mg.
The maximum dose should not exceed 7 mg/kg for non-obstetric and 3 mg/kg for obstetric epidural anesthesia.
## Pediatric Dosing
Mepivacaine is **not recommended for routine use in pediatric patients** due to limited safety and efficacy data. If used, extreme caution and individualized dosing based on weight and clinical condition are necessary. **Consult specialized pediatric anesthesia resources or the manufacturer's labeling for any available guidance.**
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. Dose reduction may be necessary due to reduced metabolism.
* **Renal Impairment:** Use with caution. Accumulation of metabolites may occur, although direct renal excretion of the parent drug is minimal.
* **Elderly or Debilitated Patients:** Reduce dose due to potential for decreased drug tolerance.
## Contraindications
* Known hypersensitivity to amide-type local anesthetics or any component of the formulation.
* Severe shock or heart block.
* Certain conditions where local anesthesia is contraindicated (e.g., infection at the injection site).
## Adverse Effects
* **Central Nervous System:** Lightheadedness, dizziness, circumoral tingling, numbness of the tongue, auditory disturbances, slurred speech, tremor, shivering, nervousness, apprehension, euphoria, confusion, drowsiness, unconsciousness, respiratory depression, arrest.
* **Cardiovascular:** Hypotension, bradycardia, cardiac arrhythmias, cardiac arrest.
* **Allergic Reactions:** Rare, but can occur.
* **Local Reactions:** Injection site pain, bruising.
## Key Drug Interactions
* **Opioids:** May potentiate sedative effects.
* **Other CNS Depressants:** Additive CNS depression.
* **Vasoconstrictors (e.g., Epinephrine):** Can prolong the duration of action and reduce systemic absorption, but may increase cardiovascular risk in certain patients.
## Monitoring
* **Vital Signs:** Heart rate, blood pressure, respiratory rate, oxygen saturation.
* **Level of Anesthesia:** Assess efficacy.
* **Signs of Systemic Toxicity:** Monitor for CNS and cardiovascular changes.
## Clinical Pearls
* Mepivacaine has a relatively rapid onset and intermediate duration of action.
* It is available with or without epinephrine. Epinephrine prolongs the duration and reduces peak plasma concentrations, but should be avoided in end-arterial areas or in patients with certain cardiovascular conditions.
* Systemic absorption can lead to serious toxicity, especially with rapid injection, large doses, or administration into highly vascular areas.
* Aspiration for blood before injection is crucial to prevent intravascular administration.
