### Overview
Mylanta is a combination antacid and anti-gas medication. Aluminum hydroxide and magnesium hydroxide are antacids that neutralize stomach acid. Simethicone is an anti-foaming agent that breaks down gas bubbles in the digestive tract.
### Primary Indications
* Relief of heartburn, acid indigestion, and sour stomach.
* Relief of pressure and bloating commonly associated with gas.
### Adult Dosing
* **Antacid/Anti-gas:** 10 mL to 20 mL orally as needed, up to 6 times daily.
* **Maximum:** Not to exceed 120 mL in a 24-hour period.
### Pediatric Dosing
* **Children 2 to under 12 years:** 5 mL to 10 mL orally as needed, up to 6 times daily.
* **Children under 2 years:** Consult a healthcare provider.
### Dose Adjustments
* **Renal Impairment:** Use with caution. Magnesium accumulation can occur. Specific dose adjustments are not established and depend on the degree of renal impairment.
### Contraindications
* Hypersensitivity to aluminum hydroxide, magnesium hydroxide, simethicone, or any component of the formulation.
* Severe renal impairment.
### Adverse Effects
* **Common:** Constipation (aluminum hydroxide), diarrhea (magnesium hydroxide), abdominal cramping.
* **Less Common:** Nausea, vomiting.
* **Rare:** Hypermagnesemia (especially in renal impairment), aluminum toxicity.
### Key Drug Interactions
* **Decreased Absorption:** Antacids can decrease the absorption of many drugs, including bisphosphonates, certain antibiotics (tetracyclines, quinolones), iron supplements, digoxin, and levothyroxine. Separate administration by at least 2 hours before or after antacids.
* **Increased pH:** May alter the absorption of drugs affected by gastric pH.
### Monitoring
* Monitor for relief of symptoms.
* Monitor for adverse effects, particularly in patients with renal impairment.
* Monitor for signs of drug interactions if patient is taking other medications.
### Clinical Pearls
* Shake well before use.
* Administer between meals and at bedtime for optimal relief of indigestion.
* Avoid concurrent use with other medications that may interact; counsel patients on appropriate separation times.
* Use with caution in patients with renal impairment due to the risk of magnesium accumulation.
