## Overview
Mylot%20at (chlorhexidine gluconate) is an antiseptic agent with broad-spectrum antimicrobial activity. It is available as a topical solution, oral rinse, and surgical scrub.
## Primary Indications
* **Skin antisepsis:** Preoperative skin preparation, hand washing.
* **Oral hygiene:** Reduction of gingivitis.
## Adult Dosing
* **Skin antisepsis:**
* **Surgical hand scrub:** Apply 5 mL to wet hands and forearms. Scrub for 30 seconds. Rinse thoroughly.
* **Preoperative skin preparation:** Apply liberally to the operative site and allow to air dry.
* **Health care personnel hand wash:** Apply 5 mL to wet hands and forearms. Scrub for 30 seconds. Rinse thoroughly.
* **Oral rinse (0.12%):** Rinse 15 mL vigorously in the mouth for 30 seconds twice daily. Do not swallow.
## Pediatric Dosing
* **Skin antisepsis:** Safety and efficacy in children have not been established for all formulations. Consult specific product labeling or local protocols.
* **Oral rinse:** Not recommended for use in children under 18 years of age due to lack of efficacy and safety data.
## Dose Adjustments
No specific dose adjustments are typically required based on renal or hepatic impairment.
## Contraindications
* Hypersensitivity to chlorhexidine gluconate or any component of the formulation.
* Avoid contact with eyes.
* Oral rinse should not be swallowed.
## Adverse Effects
* **Skin:** Irritation, allergic reactions (rare).
* **Oral:** Staining of teeth, alterations in taste, irritation of oral mucosa, dysgeusia.
* **Systemic:** Serious allergic reactions (anaphylaxis) have been reported, though rare.
## Key Drug Interactions
* **Soaps and anionic substances:** May reduce the activity of chlorhexidine. Rinse thoroughly after using soap before applying chlorhexidine.
## Monitoring
* Monitor for signs of allergic reaction.
* For oral use, monitor for staining, taste alteration, and oral irritation.
## Clinical Pearls
* Chlorhexidine has a persistent antimicrobial effect on the skin.
* Avoid contact with meninges when used for CNS antisepsis.
* Do not use to disinfect surgical implants.
* The oral rinse should not be used as a substitute for mechanical plaque removal.
